This form will allow you to ask to have a copy of an Officer’s Report which has occurred within the town of Seymour. The price for an officer’s report is $1 dollar per page and all payments must be made by Cash or Check. Report copies and other non-emergency copies will be left for pickup at Seymour Police Headquarters,
